All physical examinations must take place after May 1, 2026, for a student-athlete to participate in sports at Forest Park High School for the 2026-2027 school year. The examination results, all information, physician’s signature, stamp from physician’s office, parent’s signature, and athlete’s signature must be on this form and submitted into Arbiter.

On-Line Concussion Training PWCS

Virginia Code 22.1-271.5 requires all student-athletes and their parent/guardian to complete a concussion education program every year. The PWCS On-Line Concussion Training course is designed to be compliant with Virginia Code 22.1-271.5. It is to be completed in Arbiter.

Emergency Permission Blue Card Form

Blue cards can be picked up at the front office or outside Mr. Bricker’s office. Once completed, parent/guardian must sign the bottom of the form on both sides and write their sport in the Specialty/Designated Site Program bank on the emergency card.
